I understand that there is always an adjustment period for any adopted dog or cat coming into my home. There is no specific time limit for an animal to be in adjust, and I am willing to work to make this pet a member of my family for his lifetime. I understand that many of these dogs and cats have survived tremendous odds and needs lots of TLC. I understand that Heartsong cannot be held responsible for the actions, behaviors and/or medical condition of the dogs or cats in its Rescue. I agree to assume the risks implicit in working with dogs and cats who may have been abandoned, beaten, or otherwise mistreated or abused, or who may suffer from an illness, condition or disease. Although Heartsong evaluates dogs and cats prior to accepting them into its rescue, they cannot guarantee the temperament of any of the animals since most of their histories are unknown. ??I understand my adopted dog must be kept on a leash AT ALL TIMES when in an unfenced or unenclosed area - even when hiking, or just walking from my house to the car. All rescues are prone to being escape artists, especially in a new environment or when left alone. They may bolt out of an open front door. I will be aware and be careful. I understand my adopted cat is to stay indoors only at all times. All adopted dogs are to be treated as house pets and house/crate training is required. I understand that Heartsong reserves the right to check on the welfare of any adopted dog or cat on my premises, and to reclaim both possession and ownership without payment of any kind. In the event that I do not comply with the adoption guidelines and Heartsong reclaims any adopted dog or cat from me, I waive claim to trespasser. I understand that Heartsong must approve all adoption applications, so I will not promise anyone that they can adopt my pet in the event that I can no longer keep the pet. Of course Heartsong will be happy to receive adoption referrals and recommendations. I also agree that if, at any time in the future, I cannot continue to maintain the pet for ANY reason, I will not give away, sell or euthanize the dog or cat. I will contact the rescue coordinator for arrangements to be made to move the dog(s)/cat(s) into other foster homes with Heartsong. I agree, as is stated in the contract that I will maintain the animal as a family member with all provisions provided for until such time as a foster home can be found for the pet. I have accurately completed this application and all information stated is correct. I have read the application and program description on this form in its entirety and understand the risks involved in adopting a rescued dog or cat. I also agree that if the information given on this application is not accurate to the best of my knowledge, I forfeit all rights to any and all Heartsong adopted dogs or cats residing in my care. Your commitment to help a homeless animal enables us to rescue more dogs and cats. Thank you! Due to the time involved in processing applications please advise us if you decide you are no longer interested in adopting or would like to withdraw your application as soon as possible. This will allow us to spend our time working on open applications to find adoptive homes for our dogs and cats.
